Valjin Harvell was admitted to Northwest from Methodist Hospital following her surgery. On admission, she required moderate-maximal assistance with all functional mobility, limited range of motion and assistance with activities of daily living (ADL’s).

Functional Status on Discharge

With our seven-day clinical pathway program, Ms. Harvell was able to discharge home. There was a significant improvement in her functional mobility and ADL’s. She was modified independently with dressing, bathing, toileting, and cooking tasks. We continue to follow Ms. Valjin’s progress at home.

Mr. Reichter was admitted to Northwest from IU Methodist hospital following his surgery. On admission, he required maximal assistance with all functional mobility. He also had limited range of motion and required assistance with activities of daily living (ADL’s).

Functional Status on Discharge

With our seven-day clinical pathway program, Mr. Reichter was able to discharge home. There was a significant improvement in his functional mobility and ADL’s. He used a walker upon discharge. He is now functioning modified independence with activities of daily living (ADL’S). We continue to follow Mr. Reichter’s progress at home.
